What You Need to Bring to Your Appointment
We recommend that you print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment at Village Health Partners. Please carefully fill out the paperwork accordingly, completing Part 1 of the I-693 form, but do not sign your name until our medical team instructs you to do so. This form will be used to document and detail the results of your immigration medical exam.
Other things you’ll need:
Valid, government issued picture ID such as a driver’s license or passport (preferred).
Medical history report
I-693 paperwork
Official shot record (without proof of immunizations, you must receive all required vaccines)
Please bring an interpreter if needed.

Immigration clinic
When you register for lawful permanent status in the United States, you are required to have a medical exam or physical. The purpose of the screening exam is to make sure you don’t have certain diseases or mental disorders that would exclude you from admission to the United States. It also ensures that you are up to date on vaccinations.
The medical examination will include a medical history review, standard physical examination relevant to Immigration protocol. Blood tests for syphilis and Tuberculosis, urine test for gonorrhea and drug testing and chest x-ray may be done.
Note: The medical examination is not a complete physical examination. Its purpose is to screen for certain medical conditions relevant to U.S. immigration law.
The Immigration Medical Exam requires 4 basic steps:
- Basic medical immigration physical exam and mental status evaluation
- Blood Test:
– All adults (above age 15) are required to have a serologic blood test for Syphilis (RPR).
– All adults (above age 15) are required to have a Neisseria Gonorrhea test.
– Tuberculosis test: As of October 1, 2018 all applicants two years of age and older are required to have an IGRA test. What are the advantages of IGRAs? Requires a single patient visit to conduct the test, Results can be available within 24 hours, Does not boost responses measured by subsequent tests, Prior BCG (bacille Calmette-Guérin) vaccination does not cause a false-positive IGRA test result.
Note: If the blood test is negative and your chest x-ray is clear, the positive TST is most likely due to previous BCG vaccination. If the blood test is positive and your chest x-ray is clear, this indicates TB infection. TB germs can live in your body without making you sick; this is called latent TB infection
3. Proof of vaccinations or blood Titer to prove immunity. Most adults require proof of vaccination for:
– Tetanus-Diphtheria Td (within the last 5 years),
– Measles, Mumps, Rubella and Varicella (Chicken Pox).
– Blood Titer is a test that proves that a person has immunity or antibodies for a particular disease. The Immigration Doctor will review your vaccination history with you to determine whether you have all the required vaccinations.
– The cost of additional vaccinations is not included in the price of the basic exam but they are offered through our office for an additional fee. Please bring all vaccination records to determine the vaccines needed as part of your immigration physical.
- Once the exam has been completed, our staff, under the guidance of our civil surgeon, will complete, sign, and seal the official I-693 form, along with any supporting documents in an envelope. It is the patient’s responsibility to submit this sealed envelope to USCIS. This information is documented on the I-693 form under instructions.

Fees
Even if you have health insurance, your provider may not cover immigration medical exams, which is why San Antonio Wellness Institute doesn’t bill your healthcare provider.
